Please print and complete the following documents and bring them to your initial appointment. I have also outlined below additional things required at your first appointment.

Please wait in the main lobby waiting area when you enter the building. I will be out to get you for your appointment, as I may be with a patient.

You can opt to complete the documents prior to your appointment time. Please allow yourself 15 minutes prior to your scheduled appointment to complete the forms. The appointment time is the start of your time with me face to face to discuss treatment. The documents can be found on a clip board on the wall outside of my office and can be competed in the lobby.

Before the initial appointment and to avoid any surprises, please ask the following questions of your insurance carrier (if applicable).

• What is my annual deductible, and has it been met?
• What is my copay?
